I declare that at the time of joining this scheme, all members of my family and I are in good health and are free from any diseases that needed medical attention and were not treated for any serious condition for the last 6 (six) months. I give permission,consent to or authorize the Insurer to obtain any necessary information or any documentation that they may need to process or finalize any claim for either myself or any of my dependent members covered by this policy. I declare that all information is correct and that any willful misstatement will invalidate any claim to benefit under this policy. I acknowledge that the terms and conditions (on the reverse of this document) were explained to me and that I understand them and I undertake to abide by them. I/We accept the waiting period as contain in the master policy.
